Summary

Case 7, a 29-year-old man about 10 months into Long COVID/ME since July 2022 with extreme fatigue, PEM, tachycardia, POTS, persistent headache, brain fog, chest pain and shortness of breath, took a 15-day extended Paxlovid course in May 2023 while also on finasteride, amphetamine/dextroamphetamine salts, bupropion, atenolol, midodrine, vitamin D3 and famotidine. About six days in he had roughly 10% improvement in fatigue and brain fog that grew to about 20% by the end of the course, his longest symptom-free stretch since falling ill. Two days after his last dose he suffered a severe PEM crash and returned to his pre-Paxlovid baseline within five days, remaining there in the following months, so the gains did not hold.
